Instructions for filling Form 134 (Government Category) & Form 135 (Non Government category)

  1. Left hand thumb impression, if used, should be attested by a Magistrate or a Notary Public or a Gazetted Officer, under official seal and stamp.
  2. Deductors / Collectors are required to provide details of Assession Officer (TDS / TCS) in the application. These details can be obtained from the Income Tax Office.
  3. The deductor / collector must fill up Area Code, AO Type, Range Code and Ao Number.
  4. 'Designation of the person responsible for making payment / collection tax' field is mandatory to fill up, wherever applicable.

    Field Name

    Instructions / Guidelines

    Category of Deductor / Collector

    Applicant must select only one applicable category: Central Government, State Government, Local Authority (Central Government) or Local Authority (State Government). Incorrect selection may result in rejection of application.

    Name of Local Authority

    Mandatory where category selected is Local Authority. Name must be provided in full without abbreviations.

    Name of State

    Mandatory where category selected is State Government or Local Authority (State Government). Correct name of the State should be selected/entered.

    Name of Office

    Name of the Office must be provided in full. Abbreviations are not permitted.

    Office Address

    Address must be an Indian address and should include Country/Region, Flat/Door/Building, Road/Street/Block/Sector, PIN Code, Post Office, Area/Locality, District and State.

    Name of Organization

    Name of the Organization must be provided in full, without abbreviations.

    Name of Department

    Name of the Department should be entered in full.

    Name of Ministry

    Name of the Ministry should be entered in full, wherever applicable.

    Account Office Name

    Name of the Account Office should be provided, wherever applicable.

    Accounts Office Identification Number (AIN)

    AIN should be provided if already allotted. If AIN is not available, the field may be left blank.

    Permanent Account Number (PAN), if any

    PAN of the Government entity should be provided, if allotted. If PAN is not available, this field may be left blank.

    Official Email ID

    A valid official email ID must be provided. This email ID will be used for official communication and status updates.

    Name of Person Responsible for Deduction / Collection

    Name of the person responsible for deduction/collection of tax must be provided in full.

    Designation of Person Responsible

    Designation should be clearly mentioned (e.g., DDO, Accounts Officer, Drawing Officer, etc.).

    PAN of Person Responsible

    PAN of the responsible person must be valid and active.

    Contact Number of Person Responsible

    Country Code and Contact Number must be provided. For Indian numbers, Country Code '91' should be mentioned.

    TDS Assessing Officer Code

    Correct TDS Assessing Officer Code must be entered. These details can be obtained from the Income Tax Office.

    Declaration – Certificate Enclosure

    Certificate issued by PAO / ZAO / DTO / CDDO must be enclosed with the application.

    Declaration – Nonpossession of TAN

    Applicant must declare that the entity does not possess any TAN and shall be liable for legal consequences if declaration is found to be incorrect.

    Field Name

    Selection of Applicant Category (A to G)

    Applicant must select only one applicable category from A to G. Selection of multiple categories will lead to rejection of application.

    Registration Number

    Registration Number is mandatory for LLPs. For other entities, it may be provided if available.

    Corporate Identity Number (CIN)

    Mandatory for Companies and Branch/Division of Companies. CIN must be entered exactly as allotted by MCA.

    Name of Branch / Division

    Mandatory where application is made for a branch/division. Name must be provided in full.

    Address of Branch / Division

    Address must be complete and as per prescribed address format.

    Name of Person Responsible for Deduction / Collection

    Name must be provided in full. Abbreviations are not permitted.

    Designation of Person Responsible

    Designation must be mentioned clearly. In case of HUF, designation should be entered as "Karta".

    PAN of Person Responsible

    PAN must be valid and active.

    Contact Number of Person Responsible

    Country Code and Contact Number must be provided. For Indian numbers, Country Code '91' should be used.

     

    GENERAL INFORMATION FOR TAN APPLICANTS

    1. The fee for processing of TAN application to be paid to TIN-FCs is 77/- (Plus Goods & Services Tax, as applicable).
    2. In case the applicant has already been allotted a ten digit alphanumeric TAN, it should not apply again as having or using more than one TAN is illegal. However, different branches/divisions of a deductor/collector may apply for separate TAN for each branch/division.
    3. In case deductor/collector has already been allotted TAN and wants any change in particulars (e.g. name, address etc.), it should fill up. 'Form for changes for Correction in TAN data for TAN allotted'.
    4. Deductor/Collector will receive an acknowledgment containing a 14 digit unique number on submission of Form 49B. This acknowledgment number can be used by the deductor/collector for tracking the status of its application.
    5. As per revised Income Tax rule 2026, document referred below is required to be submitted along with FN 134 (Government Category)
          a. Certificate issued by PAO/ZAO/DTO/CDDO
    6. As per revised Income Tax rule 2026, the documents below are mandatory to be submitted along with FN 135 (for Other than Government Category):
          a. Proof of Identity (POI)
          b. Proof of Address (POA)
          c. Proof of Date of Birth / Date of Incorporation (as applicable)
